Please take a moment to fill out the following form. In an effort to be completely accurate, we are asking all parents who volunteered at the Cavalcade of Bands to submit the following form so that we can verify their volunteerism.

If you did not volunteer at the Cavalcade of Bands, sadly, we must charge your account $75.

Please complete this form ASAP!

If we don’t receive a form back, then we will assume that you were unable to volunteer that day…

Saludos PADRES…

Tómese un momento para completar el siguiente formulario. En un esfuerzo por ser completamente precisos, pedimos a todos los padres que se ofrecieron como voluntarios en la Cabalgata de Bandas que envíen el siguiente formulario para que podamos verificar su voluntariado.

Si no fue voluntario en la Cabalgata de Bandas, lamentablemente debemos cargar a su cuenta $75.

Si no recibimos un formulario, asumiremos que no pudo ser voluntario ese día...
